Here are two useful phrases from my phrasebook, Speak Natural English: 50 Must-Know Phrases for Japanese Learners.

That was my fault.
完全に⾃分の責任。丁寧で誠実な謝り⽅。

Example:
A: We’re late because of traffic.
遅れたの、渋滞のせいだよね。
B: No, that was my fault — I should’ve left earlier.
いや、僕のせいだよ。もっと早く出るべきだった。


I messed up.
やらかした・失敗しちゃった。ミスを正直に認めるカジュアルな表現。

Example:
A: Did you send the report?
レポート送った?
B: I messed up… I sent the wrong one.
やらかした… 間違ったの送っちゃった。
